Screenwriting : Format an apparition in script? by Brian O'Connor

Any suggestions on how to properly format an apparition in a script? I'd like for my lead to look out in the mist and think she sees her boyfriend. He speaks a few lines and disappears, act as her conscience. Has anyone done something similar? Thanks!
