Screenwriting : Brief plot summary by Benjamin L. Harris

Hi all, I have recently been asked to send a copy of one of my scripts along with attached release form, my problem is upon viewing the release form i must write a brief summary of theme or plot yet the space that has been allocated to write the brief is one line half the width of the page... I'm assuming by brief summary they are referring to a logline yet it clearly won't fit in a space designed for five words, am I missing something here? Should I reduce my writing size so it fits or should I attach a separate page containing the logline? I appreciate your responses. Benjamin L. Harris
