I have to agree with Beth, there are no rules. I wrote a script and i just play around with all 5 acts in my scripts. That's the best part about writing.
I love having discussions about writing, can't even find a person in singapore where we could discuss about writing:).and Dan. I what I meant about rules is about writing itself. I'm not saying about the structure. Of course formatting is important. It makes it clear about what where who etc..
In my opinion it's a fluid situation so that either you can do no wrong or you can't do anything right -- depending completely upon the reader's position and disposition.
You learn the highway code - then you drive. Then you mostly remember the rules automatically and drive, observing the rules when you want to. Same as screenwriting. BUT e.g. to put a plant in a screenplay without a payoff is just plain dumb. (etc)
all the time
not intentionally. Since I got a Rep, I try to keep pages consistent. Got to show working folks I know craft - even when I am winging it.

They're rather guidelines: good ideas, common well-known practices, things to consider. There are no "rules." ;)

there is one script rule all books preach for american movies - introduce your main Protag by page 10, or ten minutes.
