In the industry it's also known as a 'leave behind'. It generally comes into play following a face to face meeting with the Producer & Execs so that they have a better recall during an in-house preliminary greenlight meeting. I'm unaware of any format examples for you to copy but maybe someone is willing to share one. Basically, it's the logline, genre and a very brief synopsis that further demonstrates your writing capability (why you don't copy someone else's format). If you're talking about a big time FL production - then it can exceed one page (but certainly no more than 2).
