We have a celebrity that's agreed to work for SAG scale + an above scale fee schedule on a project. Their agent requires my production company to make a written offer. Does anyone have a formal offer letter template they've used and can share for name talent? Thanks.

Congratulations Kirk Sanders! I would agree with Cherelynn - you can even type up the details you know for sure in an email, shoot those over to your lawyer and have them draw up the rest of the offer to make sure it includes everything you need.
Kirk Sanders Honestly, this sounds like a time you need to bring on an entertainment attorney. Your project and the deal is likely different than anyone else's (because they all are), and you want it to be binding according to its terms (or else investor/financier's will not be impressed). It's important.
