Great topic! Also a very difficult one One adaptation that surprised me after reading the novel was the original Captain Blood. It was a really good adaptation of a novel with no clear narrative through line. The novel was very episodic with a hero who spent a lot of time feeling sorry for himself. The movie is classic in so many ways.
The Road. It was to the book, although I'd hoped they had taken the initiative in changing the end for the film. They didn't though. The film is as depressing as the book but wonderful all the same.
I'd say "Holes" by Louis Sachar. It is one of my all time favorite childhood books and films. The film stays true to the book very well, mainly because Sachar wrote the screenplay or at least was involved with the writing of it.
Perfume, great performance by Ben Wishaw. Also, In The Electric Mist (with Confederate Dead) - I think the film conveyed the tone of the book beautifully. I'm looking forward to see how well they do with Cloud Atlas when it comes out (next year?)
Richard the Third, starring Ian McKellen. It's Shakespeare, with Tanks! It's the best of the movies where they take something by Shakespeare and change when it's set, in my opinion.
Bringing up To Kill a Mockingbird reminds me of a major favorite: "Moby Dick," 1956, directed by John Huston and starring Gregory Peck. The great Ray Bradbury adapted the script from the Melville novel.

This is a hard question because film is an adaptation of it. Lord of the Rings in my opinion is one of the best.

Silence of the Lambs sticks pretty closely to the text - that book game me nightmares! Also High Fidelity - great book, great film.

I think that "To Kill A Mocking Bird" was good. Also "Tuesdays With Moorie". Just as touching on screen as in the book.
